Looking to better manage your chronic conditions (diabetes, arthritis, heart disease, etc.)? The Chronic Disease Self-Management program might be right for you. This 6-week program, available in both English and Spanish, teaches participants skills and techniques for dealing with various symptoms associated with chronic conditions, such as fatigue, stress management, and frustration.
Learn how to better manage your chronic pain through the Chronic Pain Self-Management program. This 6-week program, available in both English and Spanish, teaches participants how to better manage symptoms associated with their chronic pain, with an emphasis on cognitive behavioral therapy techniques for managing chronic pain.
Chronic Pain Self-Management (CPSMP)
Trying to slowly increase your physical activity or learn a new strategy to help manage your arthritis pain? The Arthritis Foundation’s Walk With Ease program can help you achieve your goals. This 6-week program, which can be done with a group or at your own pace, focuses on walking and increasing physical activity. It also includes health education, stretching and strengthening exercises, and strategies to help you stay motivated.

If you’re looking to better manage your chronic condition by improving your diet, Medical Nutrition Therapy might be for you. Meet with a registered dietitian and get personalized nutrition education and counseling for chronic disease management, including for diabetes, kidney disease, and heart disease.

In this 4-month program, you’ll learn strategies to track your blood pressure at home and how to eat a heart-healthy diet that works for you.
