How It Started
In 2017, Make A Choice for a Healthier Life launched as a statewide public health campaign to raise awareness about type 2 diabetes and empower Pennsylvanian communities to prevent it. Reaching tens of thousands of people, the campaign connected communities to:
The prediabetes risk test
Local lifestyle change programs
Essential health information
With more than 8 in 10 Americans living with prediabetes and unaware of their condition, this initiative filled a critical gap. It was an opportunity to reach priority populations, encourage individuals to assess their risk for prediabetes, and promote enrollment into the National Diabetes Prevention Program.

Today, Make a Choice is more than a campaign — it’s your connection to free and low-cost health programs that support chronic disease prevention and self-management.
We work with individuals, families, and community organizations to offer:
Diabetes prevention and education programs
Support for managing conditions like high blood pressure, chronic pain, and arthritis
Nutrition education and counseling
Referrals through our Health Referral Hub to connect people to the right services at the right time
We believe in meeting people where they are — whether that’s in a community center or virtually — so they can take charge of their health and thrive.

About MAKE A CHOICE
Make A Choice for a Healthier Life is led by the Training and Capacity Building Department, a team of health promotion professionals who connect communities to health resources and provide community-based organizations across Pennsylvania and Delaware with training, technical assistance, referral support, billing coordination, and other services that improve their capacity to deliver high-quality, evidence-based chronic disease programs. We are based in Southeastern Pennsylvania at the Health Promotion Council (HPC).
Our work is grounded in collaboration—with public health agencies, local partners, healthcare providers, and trusted community organizations—to increase access to health information in vulnerable communities.
About Health Promotion Council
Health Promotion Council (HPC) is a nonprofit corporation, formed in 1981, whose mission is to promote health, and prevent and manage chronic diseases, especially among vulnerable populations, through community based outreach, education, and advocacy. In 1999, the organization joined Public Health Management Corporation (PHMC), Pennsylvania’s Public Health Institute, as an affiliate to strengthen its work in the public health sector. Through partnerships, HPC convenes and fosters collaboration among community‐based organizations, government, public health professionals, health systems, and community residents.
About National Association of Chronic Disease Directors
The National Association of Chronic Disease Directors (NACDD) works to improve the health of the public by strengthening the state-based leadership and expertise for chronic disease prevention and control in states and at the national level. NACDD’s mission is national in scope, but the reach of NACDD goes beyond State Health Departments (SHD) into the communities and through partners working in local health departments, universities, other non-profit organizations, and other representatives who have self-identified as part of the chronic disease prevention workforce. A 501(c)3 non-profit organization, NACDD is a capacity building organization and for almost 30 years, has been providing support and technical assistance to SHDs, including territorial health departments. NACDD links more than 7,000 diverse, multi-disciplinary members in a network to promote national priorities, and prevention policies and programs; encourage knowledge sharing; and develop partnerships for promoting health.
